Outlook problem solved!

Turns out it's an actual bug in Outlook, as described in this Knowledge Base article. It's been fixed in an Office XP service pack that I didn't have on my system.

I have advised mamishka in email to forward that link to the publishing company. They should be able to just save her attachment out off her message. :)
